My experiences in Nicaragua and Costa Rica were absolutely unforgettable and life-changing. I was able to do and see so many things that would not have been possible anywhere else, and my spanish-speaking abilities improved tremendously. Final Reflection: Having spent two months in two different countries each with a different health care system I have learned a lot. I now understand the differences that a universal health care system has on a community and also how a universal health care system works in general. The rather unorganized system in Nicaragua at least from my perspective lead to a lot of people with inadequate or no health care. Having worked outside of Granada in the country I saw a lot of malnutrition and poorly cared children, but also adults who weren't able to pay for medications that could help them. It didn't help that a majority of the people who came to the clinic walked for an hour or more to get there. In contrast the universal health care system of Costa Rica allows for everyone and anyone to get care when they need it. On top of primary care in general, medications and basic tests are all located in the same building. The EBAIS system allows for people who live farther away from clinics/in the country to have easy access to medical care. However the only downside I have seen here in Costa Rica is the fact that the primary care in a universal health care system is a lot less personal. The high volume of patients that the doctors have to get through doesn't allow for time for complete explanations and evaluations of problems. In 10 minutes there is barely enough time to do a full evaluation and diagnosis, thus I feel that it would be easier to miss a problem or complication unless it was pretty obvious. So compared to the United States I can see that a universal health care system is a very good system because people shouldn't have to worry about getting adequate health care coverage but the high volume of patients with the shrinking number primary care physicians will be a major problem that we will face as we try to go through a health care reform.

Summary of Comparative Health Program

During my time in Costa Rica and Nicaragua I have been able to compare one of the best health systems in the world to one that is dire need of assistance. While I was in Nicaragua, I felt that the biggest issue regarding health was education. The citizens had not been given formal instruction, even on some of the most basic concerns. There is a lack of funding within the country due to years of governmental instability and corruption, but there is a chance for improving current conditions. I found that the people are very appreciative and are willing to learn if they are given the opportunity. There is always going to be issues surrounding proper funding for healthcare, as this is a concern in countries throughout the world. However, primary care (preventative care) can become significantly improved without utilizing a large portion of governmental expenditure. To fix this, there needs to be a push by the government, which is a feat that may prove to be extremely difficult in the near future.
In Costa Rica, I have found many facets of the current system to be a perfect example of what health care needs to strive for. The pharmaceutical process is efficient, population education has a stronger base structure, and there is more countrywide health coverage. There are a few problems with the universal healthcare system however, the most significant one being specialty care. There is excellent basic health care—checkups, pregnancy, and medications—but it is difficult for patients to be seen for specialty issues. The only area that gives these types of care is in San Jose, which is a long distance for many citizens to travel. Also, without an incentive for many doctors to pursue specialty practice, there is a lack of ability for expansion.
I have enjoyed this experience greatly and will take many medical aspirations and life lessons from it. Thank you very much for the opportunity to participate in this program and for all of your help!

The goal of the CRINI Internship Program is to place students, professionals and individuals in practical field research, investigation and analysis of social and environmental issues in Costa Rica in supervised internships. We offer internships in both the natural and social sciences. Examples of internship projects and locations include (but are not limited to): social work, gender issues and women´s organizations, child development and education, health care, public health, community development, businesses, sustainable development, national parks, indigenous reserves, and human rights.

These projects and internships are designed to benefit both the program participant and the host community or organization where the intern works.

Health Care Internships in both Costa Rica and Nicaragua. An eight work internship for pre-med, medical, nursing, as well as public health students.
This internship provides practical, hands-on experience in health care professionals in both countries, but also observe two different care systems and how economic conditions influence health and health care.
